During this session relevant insights for pharmaceutical care from three recently finished PhD trajectories will be presented.  They all deal with optimal pharmacotherapy, adapted to the needs of an individual patient. The diversity of topics shows the broad spectrum of pharmaceutical care as provided by hospital and community pharmacists.
