Presentation: The role of the community pharmacist in fall prevention: the ups and downs of implementation
Falls among
older people are a major healthcare problem. Medication use is an important
risk factor for falls. Pharmacists, with their expertise in medication
management, may play a key role in fall prevention. It is therefore essential
to gain insight into the opportunities to involve them more in fall prevention.
Our
research showed that patients currently have limited awareness of the role of
medication in falls. Furthermore, they expressed expectations that pharmacists
should provide information about fall-related side effects. Pharmacists in our
study reported confidence in contributing to fall prevention and, most
importantly, deprescribed fall risk-increasing drugs in approximately thirty
percent of polypharmacy patients at risk of falls. Other primary care providers
acknowledged the potential role of pharmacists but emphasized the importance of
structured collaboration.
